Plan Description and Details

The MECUDA-USA Group Life Insurance Policy has a policy design with a maximum benefit in the amount of $20,000.  The plan reduces to 50% from age 65 to 70 and 10% from age 70 and thereafter.  Members of all ages receive a 50% reduction from their scheduled death benefit in their first year of being covered under the plan.

This plan design was chosen in order to provide guaranteed insurability with no health questions to members who apply at the initial enrollment.   

A great optional children’s rider is available for families with children for only $0.60 cents per family per month regardless of the number of children.

Benefit level for each child insured is $7,500---All years

Unmarried dependent children of Eligible Members, who are U.S. residents, possess a Social Security number, and who are at least 14 days but under age 21, or 23 if a full-time student, are eligible for coverage. 

 

Key Information for MECUDA-USA Group Life Insurance Policy

• The start date for our insurance plan: July 1st.

• Open enrollment  : May 15th – June 30th.

• Once enrolled   in the group life insurance plan you will be required to fill out a Beneficiary Form.

• Premiums are paid once a year latest June 30th.

New Members (first time participants)

1. Follow the payment modalities as indicated above.

2. Provide the following biographic information (mandatory) :

• Benefits(payout amount in case of death) are paid within a  few days of completed payment request forms being received at the Hartford Insurance Company as follows :

• Age 15-65  =  $20,000.00

• Age 65-69  =  $10,000.00

• Age 70 +  =   $2,000.00

• Dependents (Children’s Rider)  = $7,500.00

• After the start date of the plan, new members can only enroll on each policy anniversary. If you are a current active member and do not renew your membership, you can apply on any subsequent policy anniversary but you will need to provide proof of insurability by answering a health questionnaire.
